Need Help. "Check Engine" light: 1215 Code

I've been having this problem for a while now... When I start my car I get no code, but eventually after driving my car the "Check Engine" light turns on. It stays on for about 10 minutes then while driving I feel a small jerk and it kicks back off.... this goes on through the whole time im driving... I've done the pedal test and it comes back with a 1215 code. I've tried to take apart my whole intake assembly all the way to the TB and re tighten everything. Every time I put it back together it seems to fix it for a couple of hours but then It comes back.

Anyone have any sugestions on fixing this? I'm hoping I dont have a bad HFM. Is my car running on a "limp mode" when the light is on?.. (why do I feel this small jerk when the car seems to reset?) sometimes my throttle response seems to be a little slower when the lights on.

1215 is Air Mass/Volume Sensor ... So it sounds like you have a sensor to replace as a first step.

what is a HFM, is it a sensor, i have it looks like a similar problem, only a littlr mild hesitation when putting on the accelerator.

Yep, its a sensor that measures air flow, usually located after the airbox (or intake). looks like this:
