I am new to the BMW world having purchased a 2004 X5 3.01 with Automatic. I woke to find the trans in failsafe mode and see there are tons of possible reasons for this.
The trans does not work in any gear and after putting it in gear and then pack to park, there is a short grinding noise (watch the video below).

Here is what I have done so far:
1 - Changed engione oil (it was planned anyway), plugs and gave it a new battery.
2 - Verified alternator voltage is acceptable (at the battery with car running and battery unhooked)
3 - Cleaned (although it looked clean already) the MAF filter and airbox (filter in airbox is like new)
4 - Changed trans filter and fluid. There was no metal in the pan at all.
5 - Using basic OBD II scanner I cleared old codes and restarted engine a few times. No codes returned; engine starts up and idles well.
6 - I got INPA working and scan of trans codes showed "code 146 engine torque signal". This was in german but I google translated it to english; I think it is right.

I did notice that the large plug connector on the trans has come un-bundled but all the wires look fine to me. I think I will need to replace this harness regardless in the near term.

So I replaced the Mass airflow sensor and cleared all the codes. Limp mode is gone now and the car goes into gear.
That is the good news. The bad news is that the car is still coding P1447, engine running rough, smells lean and does not seem to have power it should.
Smoke check the intake system
The intake boot is notorious for having splits along the accordion section. I found my split on the underside, when visibly, it had looked fine. While it helped, the thing that really made the difference was spraying the air intake temperature sensor with brake cleaner. Mine is a 2001, so your location might be different. Mine was on the top of the intake manifold, right in the middle. There is an electrical plug to remove, and then a tab to squeeze, and it will come right out. Mine was full of gunk, but it cleaned up nicely. If not, FCP Euro has them in stock. It made all the difference for me. I had already done the MAF cleaner, then replacement with minimal change.
