I’ve been shopping for steering wheels and seen regular ones and flat bottom/more of a “racing” style one.
Any positives or negatives to the flat bottom rather than just clearance?
I can't say I've ever used one, but from looking at it I feel it'd be much harder to steer with your knees while you eat breakfast.
I don't think there is any other benefit other than more leg room, and the look if that's what you're after.
I think they look cool. There is someone who can convert an E36 wheel to flat bottom and recover it.
In a road car that requires multiple rotations of the wheel to attain full lock, it seems to me that usability would suffer as the geometry changes throughout rotation.

You see them from the factory in some high performance cars so the must work fine. I had a friend with an E36 with one and drive it a few times and don’t recall any issues.
Just installed mine that I had laying around for a good 6 years. I like it.
