I noticed the bushing in the Bilstein shock was falling out. The rubber grommet on the screw side was actually covering half of the bolt head. I had to beat it back this far just to be able to get a socket on bolt head.
1- Has anyone experienced this?
2 - I'm assuming the bushing is supposed to be centered in the housing (doing so it would protrude about 1/8"either side).
3 - There was no washer on the bolt head (There will be now), nor does REALOEM indicate there should be one there.
Any thoughts?

You didn’t install the washer. Their kits include it (just about every shock manufacturer does).
Get a washer on it and continue to drive.

Make sure you also tighten that bolt with the rear back on the ground with the shock under load.

Or measure center of wheel to fender lip with car on ground, then jack the rear Spindle to that height after initial bolt installation (while car is on jack stands) and apply the final torque with Spindle jacked to ride height.
Never done it that way however, and have done several sets.

Considering the bushings slide out... it is not necessary to preload the suspension for the shock lower rubber. They likely rotate into ride height position.
Thanks everyone. I put it all back together with a big *ss washer this time. i did tighten it up on the ground. Didn't do it that way last time.
