Leak in fuel system? Smell of gas when parked, wet stain on the ground?
Someone siphoning gas from the car when you're not around? Watch the gage when you shut down and when you next start.
Smell of gas at idle? Walk slowly around the car sniffing for it, down low too, not in a windy place.
Leaking fuel line when under pressure? Look under the hood and under the car at idle, check pressure at fuel rail after shutoff.
Stuck pressure valve, check fuel pressure at idle and at higher rpm.
And the number one thing: what have you done to the car lately that has had anything to do with the fuel system? Mechanic's rule, y'know...

You probably have a vacuum leak. Smoke tests aren’t always accurate. They perform that test under very low pressure. I’ve had situations where there was a split in the intake boot and it didn’t show any smoke but the high vacuum when the car was running was enough to cause thT split to open and allow lots of air to get in. Most likely culprit is the inner intake boot

    Bank 1 only, I assume?
That should narrow down the leak locations to be bank specific.
