Has anyone else had this issue? Could it be a bad washer fluid motor?

Well, is it the tank emptying right after you fill it? Or does a big puddle of blue water appear under the car when you hit the washer fluid? Have you tested to see if the pump is getting power? Have you tested the pump with 12 Volts to see if it will come on?

No, I have not done any tests and I didn't think to have someone try to dispense fluid while the is parked...only using while driving. The tank is emptying somewhat through a tube under the hood close to blades...if that makes sense.
Yes it does in a way, that means the fluid piping is disconnected or the sprayer is clogged and needs to be replaced. The only way to know for sure is to pull the hood mat off and have a look. If you see a black tubing unhooked from the sprayers, then you have your answer. Otherwise you will need a buddy to hit the sprayers to see if the fluid comes out of the tubing.

Scroll down to the bottom. Then remove the hood liner enough to see what's leaking. Match that part to the diagram, and order that part number.
Note that all this 15 year old plastic will tend to break when you touch it; you may need additional parts, when you go to fix the leak.
