Was the inner wheel well behind the lip of the bumper or in front? The side where the lip of the bumper is behind the wheel well, the edge of the bumper always pops out as shown in the picture.

Supposed to be behind. The alignment in the last picture looks funky.

Item 5 is the part that the rear cover clips in to.
It may have a couple of broken tabs that no longer holds the cover in place
The front clips are notorious for breaking.
Yes the inner splash guard should rest inside the fender lip

I just realigned the inner wheel well and the bumper support is still good. What I did notice is that the lip on the bumper where the tabs are is deformed. It kind of drops down where the tabs are and it's not seating well in the support bracket. This is causing the bumper pop out as shown in the last photo.
Any tips to stopping this problem other than getting a new bumper?
Good chance the rear cover was damaged in an accident and the repair shop did not do a 100% repair.
This is what happened with my front cover and needed to be replaced as it kept popping out of its clips.

Ok, I came up with a fix for my rear bumper pulling away from the fender. I pulled the rear bumper off and noticed that the tabs were still there but the lip of the bumper where the tabs are were deformed and would not let the tabs snap into the bumper mount. The bumper support dose not have support for the lip were the tabs are so I created a bridge out of .032x1/2in banding material to raise the tab into the slot of the bumper support. To keep the bridge in place when I put the bumper back on I used double sided tape to affix the bridge to the support.
