Are the heated mirrors supposed to be on all the time, or only with the rear defroster?
If they are not supposed to be on all the time(which i believe is the case) is there any quick fix for ones that are on all the time? I'd like for this set not to burn out.

they are just on all the time as far as I have ever known.
it's just the way they were designed. Don't worry about them burning out.
if they do, you will only ever have to change them once on your lifetime.

did all e34's come with heated mirrors?
As far as I know...the e34 models imported to North America all came with heated mirrors AND heated door locks.
There are probably some lesser models sold in Europe (like the 518i/520i) that might not have heated mirrors.

Unlike the heated side mirrors and heated windshield washer jets (which warm up automatically)...the door lock heaters have to be initiated manually by the driver.
To initiate the door lock cylinder heater...you pull up and hold the handle for 5 seconds then let go of the handle. The lock will heat up for about 20-40 seconds (I forget the exact interval)...then you can repeat this procedure 2 more times within a few minutes if the locks are still frozen. You can only use the lock heaters 3 times within a few minutes then you have to wait like 30 minutes before it will work again. I believe info is in the owners manual.

Thanks Qsilver, you have answered all of my mirror questions, you da man :) Seems kinda crazy to have the mirrors heated up all the time, but i guess it is what it is.

I think the mirrors are actually only on if the temp is below like 40* F or somewhere around there. I thought the washer jet heaters worked in a similar fashion. Also, I dont think all e34's had heated mirrors in this country. The two 95's I have owned don't seem to, but I guess it is possible they were just broken on both cars.

is there an easy way to test the heated mirrors?
ours don't seem to do much, but granted they are normally just wet, not frosted. but i would think that the heaters would help evaporate the moisture. thanks!

The mirrors do burn out. They were burnt out on my 95 when I bought it in 2003. I still need to replace the passenger side mirror glass.

The heating element in the mirrors do eventually burn out. I had to replace mine back in 2006 and it is VERY easy to do once you find replacement mirror glass. On 1993-up models...the mirror glass just pops off and on (jut bottom of mirror out then place fingers underneath and pull)...then unplug the connection of the two wires on the back of the glass.
I think the glass stays warm all the time because the heated glass also helps in humid conditions (in the fall/summer/spring) when condensation can form on the glass...especially during foggy conditions. Think of the mirror in your bathroom when you take a hot shower...the heating element in the mirror glass will keep the side mirrors from fogging up. :)
Below is what the back of the 1993-1995 heated mirror glass looks like:

Below is how you remove the mirror glass of the older 1989-1992 style mirrors that look like the ones on the e32 7 series:

Mine always seem to be on. They've defrosted by the time I'm done scraping ice off the windows.

AdamZack, umm, this is the e34 subforum, not e39, so i advice you to ask it where the e39 guys hang out!
But i'll guess it has it by default just as the e34 has.
As far as I know...the e34 models imported to North America all came with heated mirrors AND heated door locks.
There are probably some lesser models sold in Europe (like the 518i/520i) that might not have heated mirrors.
They where an option on all e34s in europe AFAIK, do not have them on any of my e34's, which are 520 and 525 ranging from 92 to 94.
And yes, they are on at all times, just like the rear window defroster, but on low heat, when you turn the rear defroster on, it runs at full heat for a certain amount of time to then go down to "low mode".

Thanks Miche for your answer. I assumed the guy (QSilver7) with e38 and e39, who wrote about the lock heater, knew if there was that function in a e39.

qsilver doesn't really use this forum in the last couple years. As said, the E39 should have them if we did, but they will know for sure.

On some cars, one connector to the mirror is larger than the other making it impossible to wire it wrong. On the ones where each connector is the same size, I believe that it makes no difference (when you think about it, it makes sense that the heating element would function in either polarity).

Say... Would that connection plug into this maroon connector, by chance?

(I am investigating a huge E39 mystery! (https://www.bimmerfest.com/threads/purpose-of-wire-connector.668319/post-7321735) This is worth reviving an old thread, ok?)
EDIT: Just realized this is a E34 forum post. Whoops. Hello E34 owners! Though it would be interesting to know if you have the same connector in your cars.
