I may have a similar situation to yours. My car has been running terribly and idles so rough that it feels like an old 70s muscle car with a crazy cam setup. Car is throwing a 1221 code (O2 sensor) and I have hopefully narrowed it down to a vacuum leak. I sprayed carb cleaner around and under the intake and the rpm's surged when I sprayed it around the ICV inlet (right behind the throttle body on underside of intake). While I have the intake removed I am going to RTV seal in the ICV inlet hose to the intake. All hoses on ICV have hose clamps as well. Figured it couldn't hurt and if I had a leak here it would surely stop it. I'll let you know what happens when I put it back on this afternoon.

Hey man just wanted to follow up. I used RTV around the ICV hose where the plastic fitting plugs into the intake and put the fitting back in. I also have hose clamps on all the ICV connections. Fired the car up today and it ran great and CEL went off after about a minute and stayed off. If you don't want to use RTV around it like I did go pick up a new plastic piece and O ring and see if it helps. My car is running great now.
